Privacy, consent & service agreement
All WHR Allied Health clients will need to have discussed or completed our Privacy, Consent and Service Agreement before supports can commence or continue. We warmly welcome any questions about these documents — before they're completed and at any point while you're receiving supports.
Once your referral is complete, please return it (along with any relevant attachments that will help us provide the right supports) by email to [email protected], or give us a call on 03 5261 9037 to provide the details by phone.
